Collaborative Care Connection is a web-based meeting place that allows youth, family and other team members to collaborate and access elements of the Wrap-Around process, anytime of the day. Collaborative Care Connection provides families with the means to organize their team, develop connections to community-based resources, and direct interventions with a team of natural supports, professionals, and service providers.
